Financially, you don't need that much more hem-onc salary to surpass those 3yrs of hospitalist earnings, over the course of 20yrs. Hem-onc will likely out-earn hospitalist over 20+yrs cumulatively anyway. You'll be more than fine financially either way and reach your goals. Do what you want and what interests you; medically, lifestyle, etc.
After 4 years of college, 4 years of medical school, 3 years of residency, and 2 years of fellowship, you don't want to say in 20 years, man, I shouldn't have cut my training short 12 months.
